1. During negative half cycle diode is forward biased which makes diode short circuit and capacitor starts charging.
2. Hence voltage across capacitor becomes $V_C$ = 5 + 2 = 7V and output voltage $V_O$ is 2V.
3. During positive half cycle diode is reversed biased hence voltage at output terminal becomes $V_O$ = 5 + $V_C$ = 5 + 7 = 12V.
