1 Faults are caused by compressional, extensional, or lateral motion of the crust. Folds indeed are bends in the crust. They are caused by compressional forces (like pushing an area rug with your foot).
2 A fault is a large crack that appears at a transform plate boundary, at which two plates slide past each other. Folds are caused at a collision margin where two plates collide and force each other upwards.
3 Faults are along a convergent plate boundary where one plate is moving faster than the other. It does not create a depression or lead to any seismic activity, but it does shape way continents look like today, as it creates a fractured zone in between plate boundaries.
